Features Available in RevolutionPHR:

• Update changes in key information such as new phone numbers, email addresses, addresses, and more.
• Complete your pre-exam interviews online. This information automatically is added to your patient chart in our system which leads to a more accurate and efficient exam.
• Schedule appointments any time of day.
• Check the status of glasses and contact lens orders.
• See billing statements.

Access Your RevolutionPHR Patient Portal:

Our patient portal has recently been updated. Please select your preferred office below to be directed to the appropriate portal login page.

Please note: To access your portal, you must either:

• Have an existing portal account from our previous system, or
• Have a valid email address on file with our office.

If you previously used our old patient portal:

• Sign in using your existing username and password one time.
• After logging in, you will be prompted to add an email address. Future logins will use your email address and a verification code instead of your previous credentials.

If this is your first time logging in:

• Click the blue Log In button.
• Enter the email address you have on file with our office.
• Check your email for a verification code.
• Enter the code to access your patient portal.

What is RevolutionPHR?

The acronym PHR stands for “Personal Health Record” and represents an online location where you and only you can view important information about your relationship with us.

RevolutionPHR utilizes the highest standards of online security while allowing you to view portions of your eye care record through any standard Internet browser on a PC or Mac computer or an iPad. Log in using the email address you have on file with our office to securely access your patient portal and explore its many convenient features.

You can decide if access to RevolutionPHR is right for you, and participation is completely optional. We have no record of your access and we allow you to use this online portal at your discretion.
